Contractual Requirements for MLTC
Medicaid Provider Number – Notice of Regulatory Change: As of January 1st 2018, the NYS Department of Health (DOH) now requires all Licensed Home Care Services Agencies (LHCSAs) to have a Medicaid Provider Number in order to contract with Managed Long Term Care (MLTC) programs. You may have received a notification about this mandate directly from your MLTC Provider Relations Credentialing Team. It should not be ignored – as it reflects new New York State LHCSA Medicaid Enrollment Requirements for MLTC contracting.
Only LHCSA operators who have submitted a New York State Medicaid Enrollment Form through “emednyc” by the approaching DOH deadline will be considered for a NYS Medicaid Provider Number. Failure to meet the enrollment deadline will result in your LHCSA being out of compliance with the DOH statute — and you will be subject to the contract termination protocols of your MLTCs.

Historically, the DOH has a high return rate for the New York State Medicaid Enrollment Form, with 80% of forms being returned as inadequate on the first submission, 60% of forms being returned on the second submission, and 20% of forms being returned on the third.
